Grade Request Form
Please use this form when requesting a grade for a lesson or exam.
You must follow these guidelines:
You must allow up to 6 weeks from the date they were mailed for Postal Lessons or Exams to be graded.
You must allow up to 10 days from the date the lesson was uploaded for Online Lessons to be graded.
You must NOT attempt to use this form for anything other than requesting a lesson or exam grade. All other requests submitted through this form will be discarded.
